TTF-1 (Thyroid transcription factor-1) is a nuclear protein with a molecular weight of 38~40 kDa, expressed in fetal lung tissue and adult type II alveolar epithelium, but not expressed in type I alveolar epithelium. In adult tissues, it is mainly distributed in endoderm-derived thyroid follicular cells, parts of the diencephalon, and respiratory epithelium. It is positively expressed in papillary thyroid carcinoma; in lung tumors, most small cell lung carcinomas, lung adenocarcinomas, most atypical pulmonary neuroendocrine carcinomas, and some large cell undifferentiated carcinomas are positive, while lung squamous cell carcinoma and typical carcinoids are rarely or negatively expressed. It is mainly used for differential diagnosis of small cell lung carcinoma, primary lung adenocarcinoma, and metastatic adenocarcinoma, as well as for distinguishing lung cancer from malignant pleural mesothelioma and paraganglioma, and can also be used to differentiate small cell lung carcinoma from Merkel cell carcinoma. TTF-1 clones MX011 or SPT24 have higher sensitivity in tumors of lung and thyroid origin than 8G7G3/1, with cellular localization in the nucleus; however, 8G7G3/1 shows unique cytoplasmic positivity in hepatocytes and hepatocellular tumors, which MX011 and SPT24 do not, thus 8G7G3/1 can be used to differentiate primary from metastatic liver tumors.
