The IDH-1 (Isocitrate dehydrogenase 1) gene is located at 2q33. Mutations in IDH-1 are associated with brain gliomas, prostate cancer, paragangliomas, and acute myeloid leukemia. Mutations at codon 132 of IDH-1 frequently occur in WHO grade II and III gliomas. Approximately 70% of astrocytomas and oligodendrogliomas harbor IDH-1 mutations. The high frequency and distribution of IDH-1 mutations in specific brain tumors can be identified through immunohistochemical methods, such as distinguishing degenerative astrocytomas from primary glioblastomas, or differentiating WHO grade II diffuse astrocytomas from fibrillary astrocytomas or ependymomas. It can be used for the differential diagnosis between tumor margin glioma cells and reactive gliosis. Combined use with p53 can enhance the differential diagnostic capability for the aforementioned tumors.
