EMA, also known as Mucin1 (abbreviated as MUC1), is a commonly used epithelial cell marker. It is a glycoprotein found in human milk fat globule membrane, with a molecular weight of 265~400 kD, widely distributed in various epithelial cells. Its expression in glandular epithelium is superior to CK and is mainly used in the diagnosis and differential diagnosis of epithelial cell tumors. It stains positive in meningeal cells and tumors, while CK stains negative; nerve sheath meningeal cells stain positive while S-100 stains negative. It can be used for the differential diagnosis between anaplastic large cell lymphoma (EMA 95%+) and classical Hodgkin lymphoma (EMA-). In nodular lymphocyte predominant Hodgkin lymphoma, L&H cells also show positive expression (60%+). Plasmacytoma often shows positive EMA staining (65~82%). EMA can also be positive in B-cell lymphomas, such as diffuse large B-cell lymphoma and T-cell rich B-cell lymphoma. Additionally, 18% of T-cell lymphomas are EMA+. EMA is often positive in tumors such as synovial sarcoma, epithelioid sarcoma, neurogenic mucinous tumors, chordoma, and parachordoma. In the pleura, it can distinguish malignant mesothelioma (membrane expression) (82%+), adenocarcinoma (cytoplasmic expression) (51%+), and reactive mesothelial hyperplasia which is basically EMA negative. EMA is positive in ovarian granulosa cell tumors and can be differentiated from endometrial carcinoma with sex cord structures which is EMA negative.
