NSE (Neuron specific enolase) is a type of enolase dimer isoenzyme. There are generally three types of enolase dimer isoenzymes (α, β, γ). The α type is present in various tissues of mammals, the β type is found only in cardiac and striated muscle tissues, and the γ type is mainly found in nerve cells and neuroendocrine cells of the central/peripheral nervous system, as well as tumor cells derived from these cells. NSE targets the γ dimer and shows diffuse cytoplasmic staining in positive cells. However, due to cross-reactivity with the γ heterodimer, some normal smooth muscle, myoepithelial cells, renal tubular cells, lymphocytes, etc., can also express NSE, which reduces its specificity. Therefore, NSE should be used in combination with other antibodies for the diagnosis of neuroendocrine tumors to improve diagnostic accuracy.
