Uroplakin III is one of the four main members of the Uroplakin family (Ia, Ib, and II), a 47kDa glycoprotein with an asymmetric unit, and is the only Uroplakin protein with a distinct cytoplasmic domain, exhibiting high tissue specificity. It is expressed on the surface of urothelial cells in the renal pelvis, ureter, bladder, and urethra. Approximately 60% of tumors derived from human transitional epithelial cells (both primary and metastatic) show focal expression of Uroplakin III (sometimes expression may be minimal). In tumors originating from urothelial cells, loss of Uroplakin III expression is usually associated with high-grade tumors, muscle layer invasion, and lymphatic metastasis. Data indicate that Uroplakin III can be specifically expressed in p63-negative urethral epithelial tumors, including undifferentiated carcinoma; it can also be expressed in vulvar Paget's disease associated with urothelial carcinoma. Its sensitivity is lower than that of Uroplakin II. It is mainly used for the diagnosis of urothelial tumors.
