Thyroglobulin, also known as THY, is a glycoprotein with a molecular weight of 67kDa synthesized by thyroid follicular cells. It has high tissue specificity and is expressed in thyroid follicular epithelial cells and colloid. It shows positive expression in thyroid follicular carcinoma, papillary thyroid carcinoma, Hürthle cell adenoma, and nearly 50% of undifferentiated and anaplastic thyroid carcinomas. Some medullary thyroid carcinomas may show scattered expression at the tumor margins, which might be false positives caused by diffusion of THY secreted by surrounding thyroid tissue. Combined use with CT (calcitonin) is used for identifying medullary thyroid carcinoma. THY generally does not react with epithelium of organs such as lung, breast, gastrointestinal tract, pancreas, kidney, and others where tumors occur. It is mainly used for diagnosis and differential diagnosis of various types of thyroid cancer and metastatic adenocarcinoma. Combined use with TTF-1 is used to differentiate primary thyroid tumors from lung tumors.
