INSM1 belongs to the zinc finger transcription factors, which are DNA-binding proteins. The gene is located at 20p11.2 and has no intron structure. It contains five zinc finger modification sites and multiple functional regions. It is highly expressed in neuroendocrine tissues and the nervous system during mammalian development and plays an important role in early embryonic neurogenesis. Compared to neuroendocrine antibodies (CD56, CgA, Syn), INSM1 has higher sensitivity and specificity than CD56 and Syn, is localized in the cell nucleus, and is easier to interpret.
INSM1 is expressed in endocrine organs and gastrointestinal neuroendocrine cells.
INSM1 is highly expressed in pulmonary neuroendocrine tumors (the 2023 CSCO guidelines have added INSM1 as an immunohistochemical marker for small cell lung cancer).
It is used for differential diagnosis between carcinoid tumors and pulmonary meningioma-like nodules.
High expression of INSM1 is associated with increased progression-free survival and chemotherapy sensitivity in small cell lung cancer.
