FH is an enzyme located on chromosome 1q43 that catalyzes the reversible conversion of fumarate to L-malate in the TCA cycle. The cytoplasmic form of FH is involved in the metabolism of amino acids and fumarate and can translocate to the nucleus to repair damaged DNA. 2SC (2-succinylcysteine) is a product of FH loss of function. Combined with 2SC, it is used to diagnose hereditary leiomyomatosis and renal cell cancer (HLRCC) syndrome: FH immunostaining loss in cutaneous leiomyomas and positive 2SC expression are sensitive and specific markers for HLRCC.
