SF-1 (Steroidogenic factor-1), also known as Ad4BP or NR5A1, is a member of the orphan nuclear receptor family. It plays a key role in steroidogenesis, regulation of brain and pituitary hormones, and the development of adrenal tumors. It is an important regulatory factor for the development and function of the adrenal glands and gonads. It is mainly expressed in the pituitary, hypothalamus, adrenal glands, supporting and stromal cells of the testes, and granulosa and theca cells of the ovaries. In the diagnosis of pituitary adenomas, SF-1 is a marker for gonadotroph pituitary adenomas. The expression of SF-1 varies in benign and malignant adrenal tumors; for example, it is highly expressed in non-functional adrenal cortical adenomas and aldosterone-producing carcinomas, but lowly expressed in cortisol-producing tumors, with a positive correlation to tumor size. It can be used for differential diagnosis between adrenal cortical tumors, pheochromocytomas, and renal cell carcinomas.
