SDHB (Succinate dehydrogenase B) is located on the inner mitochondrial membrane and can convert succinate to fumarate in the tricarboxylic acid cycle. SDH consists of four protein subunits (A, B, C, D), and mutations in any subunit can lead to functional instability or even loss. SDHB is expressed in tissues such as renal tubules, salivary glands, placental trophoblasts, and neurons of the cerebrum and cerebellum. It is mainly used to detect SDHB mutations in gastrointestinal stromal tumors (GIST), pheochromocytomas, and some renal cell carcinomas. SDH-deficient GIST shows negative results and is clinically often associated with Carney triad (GIST, paraganglioma, and pulmonary chondroma) or Carney-Stratakis syndrome (familial GIST and paraganglioma).
Caution is needed when interpreting staining results: (1) The staining pattern is distinctly granular positive in the cytoplasm (mitochondrial staining); (2) Internal controls are often present in the stained samples, meaning non-tumor cells such as endothelial cells, stromal cells, or inflammatory cells should be stained.
