CR is an intracellular calcium-binding protein with a molecular weight of 31.5 kD, belonging to the troponin C superfamily. This type of protein is characterized by containing a structure known as the EF-hand region and is mainly found in nervous tissue. CR+ is present in the retina, the keratinized superficial layer of hair follicles, exocrine glands, renal tubules, the high secretion phase of the endometrium, mast cells in the uterine myometrium, adrenal cortex, supporting stromal cells of the testis, keratinized epithelial cells of the thymus, and adipocytes.
9.8% of adenocarcinomas can locally express CR+, while the vast majority of mesotheliomas are positive, especially epithelial mesotheliomas, with sensitivity up to 100% and specificity of 50%. Therefore, CR is one of the best markers for the diagnosis and differential diagnosis between adenocarcinoma and mesothelioma.
Cardiac myxomas (with strong and diffuse nuclear and cytoplasmic positivity), adrenal cortical tumors, adenoma-like tumors, ameloblastomas, myofibroblastomas, synovial sarcomas, and papillary carcinoma of the thymus often show CR+. CR is also widely expressed in squamous cell carcinoma. Transitional cell carcinoma of the bladder stains negative.
CR can be used in combination with Inhibin for the differential diagnosis of sex cord-stromal tumors (except fibrous follicular membrane cell tumors). In granulosa cell tumors, the positive rate of CR (90%) is higher than that of Inhibin (60%); in Sertoli-Leydig cell tumors, Inhibin (70%) has a higher positive rate than CR (57%); in sex cord-stromal tumors with abundant spindle cell/fibrous components, Inhibin (43%) is better than CR (14%); in follicular membrane cell tumors, both have the same positive rate. It is noteworthy that both can show positive reactions in ovarian surface epithelial tumors.
CR expression varies in colorectal cancers of different differentiation levels: well-differentiated 5%+, moderately differentiated 20%+, poorly differentiated 67%+.
New neuronal and nervous system markers show that the disappearance of submucosal ganglion cells supports the diagnosis of megacolon; if CR-positive ganglion cells are present, the diagnosis cannot be made.
