Berep.4 is a cell membrane surface glycoprotein with unknown function, widely distributed in epithelial cells (except for superficial squamous epithelium, hepatocytes, and gastric parietal cells), and shows positive staining in the distal tubules of the kidney.
Its main application is in the differential diagnosis of adenocarcinoma (over 80%) and mesothelioma (over 12%). Adenocarcinoma cells show strong and diffuse cytoplasmic/membranous staining; mesothelioma shows few positive cells with weak and focal cytoplasmic staining or negative reaction.
Berep4 is negative in hepatocellular carcinoma but positive in cholangiocarcinoma and metastatic adenocarcinoma.
Berep4 is positive in papillary renal cell carcinoma but negative in clear cell renal carcinoma, which can be used in combination with other renal cell carcinoma markers (such as CD10) for differential diagnosis.
Berep4 is positive in connective tissue proliferative small round cell tumors.
It also assists in the diagnosis of hereditary nonpolyposis colorectal cancer.
