CD15 antigen is also known as X-half antigen. It stains positive in granulocytes (best positive internal control) and in cells infected with cytomegalovirus (CMV). Almost all chronic myeloid leukemia (CML) cases are CD15+, whereas acute lymphoblastic leukemia (ALL) rarely expresses it. Reed-Sternberg (R&S) cells in classical Hodgkin lymphoma are CD15+ (80%), while lymphocyte and histiocyte (L&H) cells in nodular lymphocyte predominant Hodgkin lymphoma often stain negative. About 5% of B-cell lymphomas are CD15+, but most show focal and weak positive expression. A few anaplastic large cell lymphomas are CD15+.
Some histiocytes and histiocytic tumors are CD15+ (such as Langerhans cell histiocytosis).
CD15 can be used in the differential diagnosis of adenocarcinoma (68%+) and mesothelioma (7%+).
In Hodgkin-like lesions, the expression pattern of cells is CD15-/CD20+/CD30+.
