Oct3/4 (Octamer binding transcription factor 3/4) is a transcription factor with a nuclear POU domain, located on chromosome 6p21.3. Oct3/4 is a nuclear protein that plays an important role in maintaining the pluripotency of embryonic stem cells. It is expressed in undifferentiated embryos or pluripotent stem cells and is downregulated in all differentiated somatic cells. It is positive in seminomas, dysgerminomas, and embryonal carcinomas, while yolk sac tumors, spermatocytic seminomas, and choriocarcinomas are negative. Positive expression can also be seen in precursor lesions of germ cell tumors and intratubular germ cell neoplasia unclassified (IGCNU). In a few clear cell carcinomas, Oct4 may show focal positivity, which should be noted in the differential diagnosis between clear cell carcinoma and dysgerminoma. It is mainly used for the diagnosis and differential diagnosis of germ cell tumors.
