CD20 antigen is a non-glycosylated phosphoprotein with a molecular weight of approximately 33kD. It is a broad-spectrum B lymphocyte marker, expressed from immature B cells until the terminal stage of B cell development (before plasma cell differentiation).
CD20 is highly sensitive (>95% of B-cell lymphomas express it positively), although staining in B-cell chronic lymphocytic leukemia (B-CLL/SLL) is often weak (and may be negative if the detection system is not sensitive enough). About 50% of lymphoblastic lymphomas are negative.
CD20 staining is basically negative in plasma cells, plasmacytomas, and plasmacytoid lymphomas. Therefore, CD20 expression can provide useful clues for plasmacytomas and plasmacytoid lymphomas.
In thymomas, epithelial cells with a distinctive star-shaped morphology are CD20+ (the only non-lymphocyte positive cells, CKpan+), which is often a diagnostic clue for ectopic thymomas or thymomas, especially positive in spindle cells (type A thymoma). If CD20-/CD5+ is observed, it can help diagnose thymic carcinoma.
In B-cell lymphoma cases treated with anti-CD20 (rituximab) antibody therapy, CD20 may become negative; in such cases, other B-cell lineage markers such as CD79a or Pax-5 should be used.
A very small number of pleomorphic T-cell lymphomas also express CD20 and CD79a, which is uncommon but can be a diagnostic pitfall.
