The CD3 molecule is composed of 5 different polypeptide chains, with a molecular weight of 16-28kD. It is highly sensitive and specific to T lymphocytes and is not expressed in B lymphocytes; some activated NK cells are positive, showing strong staining in the perinuclear region, often covering the nuclear folds. Tissue cells can digest CD3 released by T cells (showing a horseshoe-shaped staining pattern in the cytoplasmic granules). More than 90% of T or NK cell lymphomas are CD3 positive. A very small portion of T lymphocyte lymphomas are CD3 negative, such as anaplastic large cell lymphoma, mycosis fungoides, and polymorphic lymphomas.
