Napsin A (Aspartic proteinase napsin, 天冬氨酸蛋白酶A) is a single-chain protein with a molecular weight of approximately 38 kDa. It is primarily expressed in the lungs and kidneys, with low expression in the spleen. In the lungs, it is found in normal type II alveolar epithelial cells and participates in the production of surfactant protein B. Alveolar macrophages contain Napsin A due to phagocytosis. In the kidneys, it is expressed in the proximal tubular epithelium and is involved in lysosomal protein degradation metabolism. The sensitivity of Napsin A expression in lung adenocarcinoma is significantly higher than that of SP-A and SP-B, and it exhibits very high specificity. In some poorly differentiated lung adenocarcinoma cases where TTF-1 is negative, Napsin A is usually positively expressed. Therefore, it is recommended to use Napsin A in combination with TTF-1 to differentiate primary lung adenocarcinoma from adenocarcinomas originating from other tissues or organs. It can also be used together with Pax-8 for identifying kidney-origin tumors and with HNF1β for diagnosing ovarian clear cell carcinoma.
