ERG belongs to the EST-family transcription factors, continuously expressed in endothelial cells, regulating endothelial cell differentiation, angiogenesis, the expression of certain specific endothelial antigens, and the differentiation of embryonic stem cells into endothelial cells.
ERG is expressed in hepatic angiosarcoma, with higher sensitivity than CD31, CD34, and factor VIII-related antigen, and is not expressed in undifferentiated carcinoma, sarcomatoid carcinoma, and GIST [2]. It has significant value in the diagnosis of angiosarcoma.
ERG detection shows high sensitivity for EWSR1:FLI1 and EWSR1:ERG rearrangements in Ewing sarcoma, helping to distinguish it from other small round cell tumors. It is widely expressed in epithelioid sarcoma but lacks specificity.
The positive rate of ERG in prostate cancer is 40%-50%, indicating TMPRSS2-ERG fusion. Its sensitivity is lower than P540S, but it has higher specificity and is not expressed in benign prostate tissue.
