CDK4 is a cyclin-dependent kinase, with its gene located on the long arm of chromosome 12. Due to the common amplification of the long arm of chromosome 12 in well-differentiated liposarcoma and dedifferentiated liposarcoma, multiple oncogene products in this region are highly expressed, including MDM2 and CDK4. However, myxoid liposarcoma and pleomorphic liposarcoma, due to different genetic alterations, almost never exhibit this immunophenotype.
Primarily used in conjunction with MDM2 to diagnose well-differentiated liposarcoma and dedifferentiated liposarcoma, the diagnostic value is higher when both are positive simultaneously (the rate of simultaneous positivity for MDM2 and CDK4 immunohistochemistry is 90.9% in well-differentiated liposarcoma, 90.2% in dedifferentiated liposarcoma, 2.3% in benign adipocytic tumors, and 3.7% in other sarcomas).
Combined detection of MDM2 and CDK4 is mainly used to differentiate well-differentiated liposarcoma from spindle cell/pleomorphic lipoma or classic lipoma, as well as to distinguish dedifferentiated liposarcoma from other high-grade sarcomas. It is especially helpful for diagnosis in needle biopsy specimens.
