MGMT is a DNA repair enzyme that can repair DNA alkylation damage caused by nitrosourea chemotherapy drugs, making tumor cells resistant to nitrosourea drugs. It has a molecular weight of 25 kD. It is highly expressed in the liver in normal tissues, and lowly expressed in brain tissue and bone marrow.
Tumor cells can be divided into two types based on MGMT activity: ① Tumor cells with high MGMT activity (Mer+ type) can repair DNA damage caused by nitrosourea drugs and are resistant to nitrosourea drugs; ② Tumor cells with low MGMT activity (Mer- type) cannot effectively repair DNA damage and are sensitive to nitrosourea drugs. Mer- type tumors can be directly treated with nitrosourea drugs; for Mer+ type tumors, related drugs need to be used first to inhibit MGMT activity, followed by treatment with nitrosourea drugs.
Studies have shown that MGMT mRNA, protein, and enzyme activity are closely related, and detecting MGMT protein can directly reflect enzyme activity. Measuring MGMT enzyme activity can guide clinical chemotherapy regimens for tumor patients with different MGMT activities, improving the efficacy of nitrosourea drugs. Tumors with low MGMT expression can be treated with alkylating chemotherapy drugs, while those with high expression are resistant.
