CDX2 (Caudal type homeobox transcription factor 2) is an intestine-specific transcription factor expressed early in intestinal development, involved in regulating the proliferation and differentiation of intestinal epithelial cells. It is expressed on the surface of villi and crypts of the intestinal epithelium, from the duodenum to the rectum, including absorptive cells, goblet cells, Paneth cells, and endocrine cells. Loss of CDX2 promotes the development of sporadic colon cancer and is a significant feature of large cell poorly differentiated colon cancer; CDX2 expression is closely related to the occurrence of gastric cancer, detectable in 74% of gastric cancers. Additionally, CDX2 plays an important role in gastrointestinal epithelial metaplasia and tumorigenesis. It is used in combination with Villin to differentiate colorectal adenocarcinoma from metastatic adenocarcinoma.
