Desmin is a cytoplasmic protein, an intermediate filament, located near the Z-line of the sarcomere, present in smooth muscle, skeletal muscle, cardiac muscle, and their tumors.
Applications:
1. Differential diagnosis of spindle cell tumors, serving as a marker for myogenic tumors (smooth muscle tumors and rhabdomyosarcomas), although only about 70% of leiomyosarcomas show positive reactions;
2. The perinuclear "comma-shaped" positive staining has good specificity for diagnosing small round cell tumors with connective tissue proliferation;
3. Some tumors with fibroblast components also express Desmin.
