CD138, also known as Syndecan 1, is encoded by a gene located on chromosome 2q23-24. It is an important regulator of cell and cell/extracellular matrix interactions, especially regulating cell morphology and adhesion. It is expressed in early precursor B cells and re-expressed upon differentiation into plasma cells. At the same time, CD138 is expressed in many epithelial tissues. Squamous epithelial cells show strong cytoplasmic staining. Mature mesenchymal and neural tissues are not stained. CD138 is expressed in almost all plasma cell malignancies. In diffuse large B-cell lymphoma, CD138 positivity is associated with a poorer prognosis. It is also expressed in skin squamous cell carcinoma and basal cell carcinoma, colorectal cancer, cholangiocarcinoma, transitional cell carcinoma, endometrial adenocarcinoma, breast cancer, hepatocellular carcinoma, renal cell carcinoma, and lung adenocarcinoma. Expression of plasma cells in tissue specimens may be a useful auxiliary means for diagnosing chronic inflammatory conditions (such as endometritis).
